Sodium Bicarbonate Injection is a sterile, hypertonic solution administered intravenously by healthcare professionals to correct metabolic acidosis, balance blood pH, and treat certain types of drug intoxications. It acts as an electrolyte replenisher and systemic alkalizer.

Uses
Sodium Bicarbonate Injection is used in various critical medical situations: 
  • Metabolic Acidosis: Treating high acid levels in the body (acidosis) resulting from conditions like severe renal disease, uncontrolled diabetes, shock, or severe dehydration.
  • Cardiac Arrest: Used in prolonged cardiac arrest cases, typically after other resuscitation methods have been attempted and adequate ventilation established.
  • Poisoning and Drug Intoxication: Increasing urinary pH to speed up the removal of certain toxic substances from the body, such as in poisoning by salicylates or methyl alcohol.
  • Severe Diarrhea: To replenish salt and electrolyte levels lost during severe diarrhea accompanied by a significant loss of bicarbonate.
  • Hyperkalemia: Helps reduce high potassium levels in the blood, especially if associated with metabolic acidosis. 
Potential Side Effects
Side effects typically occur with overly aggressive therapy or in patients with pre-existing conditions. 
  • Common side effects: Injection site reactions (pain, swelling, redness), increased thirst, and stomach cramps.
  • Serious side effects:
    • Metabolic Alkalosis: Excessive blood alkalinity, which may lead to muscle twitching, irritability, and tetany (muscle spasms).
    • Hypernatremia: High blood sodium levels.
    • Fluid Overload: Can cause edema (swelling) or pulmonary edema (fluid in the lungs) in susceptible patients.
    • Extravasation: Inadvertent leakage into surrounding tissue at the injection site can cause chemical cellulitis and tissue damage. 
Important Precautions
  • Medical Supervision: Requires close monitoring of blood acid levels, electrolytes (sodium, potassium, calcium), and kidney function.
  • Contraindications: Avoid use in patients with alkalosis, hypocalcemia (low blood calcium), kidney failure, heart failure, or conditions involving sodium retention.
  • Drug Interactions: Sodium Bicarbonate Injection is incompatible with many other intravenous medications, including those containing calcium, and should not be mixed in the same line or added to the same solution to avoid precipitation or inactivation.
  • Pediatric Use: Rapid injection in neonates and children under two years of age is not recommended due to the risk of intracranial hemorrhage and hypernatremia; slow administration of a diluted solution is preferred.
